In Law No. 6698 on the Protection of Personal Data ("PDP Law"),

the owner of personal data (hereinafter referred to as the "Applicant"), who is defined as the data owner,

is granted the right to make certain requests regarding the processing of his or her personal data in accordance with Arle 11 of the PDP Law.

Pursuant to the first paragraph of Article 13 of the PDP Law;

regarding these rights must be made to us in writing or by other methods determined by the Personal Data Protection Board ("Board").

In this context, requests to be made "in writing" to the Company may be submitted to us by printing this form, signing it with the following

• "Secure Electronic Signature" as defined in the Electronic Signature Law No. 5070 by the Applicant

In accordance with Article 13, Paragraph 2 of the PDP Law,

your requests submitted to us will be responded to within thirty (30) days from the date of receipt of your request, depending on the nature of the request.

Our responses will be provided to you in writing or electronically in accordance with Article 13 of

the relevant PDP Law.
